SmartHealth is your voluntary wellness program that supports whole person well-being. Participate in activities that support all of you, including managing stress, building resiliency, and adapting to change. 

As you progress on your wellness journey, you can qualify for a SmartHealth wellness incentive.

Who is eligible?

Generally, subscribers and their spouse or state-registered domestic partner enrolled in a SEBB medical plan can participate in SmartHealth.

However, only the following subscribers enrolled in a SEBB medical plan can qualify for the SmartHealth wellness incentive.

  • Employees
    Are you an employee waiving SEBB medical coverage? If so, you can still access SmartHealth but you won't be eligible to qualify for the SmartHealth wellness incentive. 
  • SEBB Continuation Coverage (Unpaid Leave) subscribers
  • SEBB Continuation Coverage (COBRA) subscribers

What is the incentive?

SmartHealth offers a $125 wellness incentive. To get your $125 incentive, you must be enrolled in a SEBB medical plan as a subscriber in 2022.

How you receive your $125 incentive depends on the type of medical plan you enroll in:

  • High-deductible health plan: A one-time deposit of $125 into the subscriber's health savings account (HSA) in January 2022.
  • All other plans: A $125 reduction to the subscriber's 2022 SEBB medical plan deductible in January 2022.

How do I qualify?

Complete these steps by the deadlines described below to qualify for the $125 wellness incentive.

  1. Sign in to SmartHealth.
  2. Complete the SmartHealth well-being assessment. This step earns 800 points.
  3. Join and track activities to earn at least 2,000 points.

Do you need an accommodation? If you are unable to complete a wellness incentive requirement, contact Limeade (SmartHealth administrator) at 1-855-750-8866. They will work with you to provide an alternative requirement that will allow you to qualify for the wellness incentive or waive the requirement. What are the deadlines?

Your deadline for the $125 wellness incentive depends on your SEBB medical plan effective date. Use the table below to find your deadline.

Current SEBB medical plan subscriber November 30, 2021
New SEBB medical plan subscriber with a medical effective date in January through September 2021 November 30, 2021
New SEBB medical plan subscriber with a medical effective date in October through December 2021 December 31, 2021

How do I find my $125 wellness incentive?

If you qualified for the incentive in 2020, look for the "How do I find my $125" activity in SmartHealth. It shows how your incentive is applied. The incentive will be applied in January 2021. You can also visit your medical plan's member portal or contact your SEBB medical plan.
