EMPLOYEE CHANGE NOTICE

Use this form to change your name, address, telephone and/or emergency contact information. Please note, requests for name change require the HR Department to inspect your new Social Security Card and Driver's License reflecting the name change.

SEPARATION NOTICE

If you desire to separate from your employment with the District, we request that you complete this form and return it to Human Resources.

LEAVE OF ABSENCE REQUEST

Use this form to request a leave of absence. Please be sure to refer to your specific Collective Bargaining Agreement  for impacts of leave upon salary, benefits, job status, etc.

This form is used by supervisors and hiring managers. If you need to hire someone, transfer an employee to a new assignment or make changes to a position, this is the form you'll need.
